Listening to reports and obtaining assignments at the start of your shift to understand each resident's current care plans;
Providing personalized care, including assistance with daily living activities like bathing, grooming, dressing, and mobility;
Participating in collaborative care planning meetings with the clinical support team to create resident-centered care plans;
Documenting care accurately in our electronic medical record system;
Preparing and serving meals, snacks, and cleaning up afterward, ensuring the preferences of each elder are met
