About Veterans Home at HolyokeThe Veterans Home at Holyoke is a state‑funded, fully accredited health care facility that offers veterans quality health care, full‑time residential accommodations, and educational and social services. It employs approximately 330 staff members, including administrators, clinicians, nurses, certified nursing assistants, skilled trade, dietary, housekeeping, and other professionals. Position OverviewWe are seeking compassionate, professional Licensed Practical Nurses (LPNs) to supervise an assigned unit and foster teamwork across multiple disciplines. LPNs will provide patient‑driven care and support to our veteran population, working both as a team member and independently, communicating clearly with patients, families, and team members.
Duties and Responsibilities- Provide nursing care to veteran residents by assessing health status, responding to call lights or veteran requests, and making observations during rounds.
- Administer nursing treatments, procedures, doctor orders, and care plans; observe, identify and respond to needs for care, including medication, equipment‑assisted care and patient/family education.
- Identify, evaluate and assess veteran health status; collaborate with supervising Registered Nurses (RNs) and medical staff for treatment modalities.
- Recognize urgent or emergency patient care situations; seek assistance of the RN and/or medical staff and initiate appropriate emergency interventions as directed.
- Collaborate with resident services personnel to ensure that individual care plans are followed.
- Establish constructive relationships with veterans and their families to promote positive relationships and learning as outlined by the treatment/teaching plan.
- Document incidents with or without adverse effect to the veteran and refer to RN for follow‑up and/or investigation.
- Foster the Veterans Home’s mission, vision and operational policies while maintaining compliance with Joint Commission and state/federal regulations.
- Maintain personal and professional growth by attending in‑services and staying current with healthcare trends, practices and Veterans Home committees.
- Use computers as needed for completing work assignments.
- Part‑time (24 or 32 hours per week) positions available on Evening (3:00 pm – 11:30 pm) and Night (11:15 pm – 7:15 am) shifts with alternating weekend and holiday schedules.
- Related to the 24/7 long‑term care facility, presence during nontraditional hours may be required when circumstances dictate operational need.
- Current, active license as a practical nurse under the Massachusetts Board of Registration in Nursing.
- CPR certification through the American Heart Association.
- Ability to follow oral and written instructions.
- Ability to deal tactfully with others.
- Certification in dementia, hospice or other relevant discipline.
- Ability to establish effective relationships with individuals, families and team members.
- Demonstrated attention to detail with readiness to respond appropriately when needed.
- Capacity to maintain accurate records and exercise discretion with confidential information.
- Computer proficiency for completing work assignments.
This position is designated as an emergency personnel role. During a declared State of Emergency and in non‑emergency adverse weather conditions, mandatory attendance may be required.
