This is an open and continuous posting for part‑time positions on our Evening (3:00 pm – 11:30 pm) and Night (11:15 pm – 7:15 am) shifts with an alternating weekend and holiday schedule. The Veterans Home at Holyoke seeks compassionate, professional LPNs to supervise within an assigned unit and foster teamwork across disciplines.
Duties and Responsibilities- Provide exceptional nursing care to veteran residents by assessing health status, responding to call lights or veterans’ requests for assistance, and making observations on rounds.
- Administer nursing treatments, procedures, doctor orders, and care plans; observe, identify and respond to the needs for care, including medication, equipment‑assisted care and patient/family education.
- Identify, evaluate, and assess veteran health status; confer with supervising RNs and assigned medical staff for treatment modalities.
- Recognize urgent or emergency patient care situations; seek assistance of the RN and/or medical staff and initiate appropriate emergency interventions as directed or needed.
- Collaborate with other resident services personnel to ensure that residents’ individual care plans are being followed.
- Establish constructive relationships with veterans and their families to promote positive relationships and learning as established by the “treatment/teaching plan.”
- Document incidents with or without adverse effect to the veteran and refer to RN for follow‑up and/or investigation.
- Foster the Veterans Home’s mission, vision and operational policies while maintaining regulatory and department compliance with Joint Commission, and state/federal regulations.
- Maintain personal and professional growth by attending in‑services, keeping current with healthcare trends and nursing practices, and participating in Veterans Home committees.
- Use computers as needed for the completion of work assignments.
- Current, active practical nurse license under the Massachusetts Board of Registration in Nursing.
- CPR certification through the American Heart Association.
- Ability to follow oral and written instructions.
- Ability to deal tactfully with others.
- At least one year of experience in caring for geriatric patients in a long‑term or similar setting.
- Certification in dementia, hospice, or related discipline.
- Ability to establish effective relationships with individuals, families, and team members.
- Demonstrated attention to detail with readiness to respond appropriately when needed.
- Capacity to maintain accurate records and exercise discretion in handling confidential information.
- Computer proficiency for completing work assignments.
This position is designated as an emergency personnel position. During a declared State of Emergency and in adverse weather, employees may be required to report to work.
