• Listen to reports and obtain assignments at the start of your shift to understand each resident’s current care plans.
• Provide personalized care, including assistance with daily living activities like bathing, grooming, dressing, and mobility.
• Participate in collaborative care planning meetings with the clinical support team to create resident‑centered care plans.
• Observe residents closely and promptly report any changes or reactions to the nurse.
• Document care accurately in the electronic medical record system.
• Prepare and serve meals, snacks, and clean up afterward, ensuring each resident’s preferences are met.
• Assist with laundry and maintain a clean, welcoming home environment.
• Communicate effectively with co‑workers to meet the needs of residents and ensure their safety and engagement.
• Attend team meetings and actively contribute to discussions.
• Report accidents and incidents, follow safety protocols, and maintain a hazard‑free environment.
• Participate in educational programs and maintain required certifications.

• Maintain a Certified Nurse Assistant (C.N.A.) certification as required by the State of Massachusetts.
• Receive training in safe food handling techniques, CPR, First Aid, and Dementia Care.
• Demonstrate culinary skills for meal preparation.
