Responsibilities

Working as part of a multidisciplinary outreach team, the RN will provide person‑centered, evidence‑based services focused on independent living for individuals with psychiatric and substance use needs. Key functions include:

  • Assess health status and coordinate comprehensive medical and psycho‑physiological care.
  • Participate in medical reviews and develop treatment plans.
  • Offer counseling, skill building, and behavioral health monitoring.
  • Collaborate with clinicians, housing specialists, substance use specialists, and community providers.
  • Maintain accurate documentation and facilitate communication among team members.
  • Serve an administrative role overseeing health promotion and medication management.
Requirements
  • Bachelor’s degree preferred.
  • Massachusetts Registered Nurse license required.
  • Minimum two years of experience in mental health and/or substance use settings.
  • MAP (Medication Assisted Treatment) experience preferred.
  • Residential experience preferred.
  • Clean driving record acceptable.
Salary & Benefits

Annual salary: $85,000 plus a comprehensive benefits package that includes Dental, Health and Life insurance, paid time off, earned vacation time, and paid holidays.
