Lead CNA – Evening (CNA III)

The Massachusetts Veterans Home (MVH) at Holyoke is seeking an experienced and collaborative professional for the role of Lead CNA (Nursing Assistant III). The incumbent works under the direction of RNs and LPNs to provide non‑clinical and essential patient care to adult and elderly veterans, many of whom have chronic physical, psychological and emotional disabilities and communicable diseases.

Full‑time (40 hours per week) from 3:00 pm to 11:30 pm with an alternating weekend and holiday schedule.

• Provides and documents bedside care to veterans based on their plan of care and unique needs, and reports outcomes of care.
• Provides daily patient care including bed/tub/bath care, mouth care, skin care, foot care, incontinent care, catheter care, vital signs, weights and ADLs.
• Transfers, ambulates, maintains proper positioning and body alignment, and performs passive range‑of‑motion exercise.
• Follows each veteran’s bowel/bladder program.
• Reports and documents all patient care activities, particularly changes in condition, skin, behavior or habits.
• Demonstrates effective problem‑solving by utilizing good judgement and interpersonal skills to promote a professional, safe and productive environment.
• Reports complaints by patients or family members to supervisors.
• Responds to emergencies.
• Transports patients to the dining room and other activities.
• May accompany veterans to medical appointments off the Veterans Home campus.
• Maintains competencies in all aspects of patient care, documentation, procedures and equipment to ensure delivery of safe care and compliance with long‑term care regulatory standards.
• Communicates with other departments, participates in inter‑shift reports and contributes at staff meetings.
• Skilled in the use of computers, email, shared network folders and contemporary nursing software including an Electronic Medical Record (EMR).
• Assists with recreational activities.
• Provides support and guidance to CNA I’s and CNA II’s, ensuring consistent adherence to policies, procedures and standards of care.
• Facilitates staff scheduling for in‑services and training.
• Participates in interview panels for new applicants, completing interview grading forms.
• Completes assigned audits related to CNA documentation and submits results to designated leaders.
• Assists with orientation of newly hired CNAs to ensure consistency with policies, procedures and standards of care; helps create and maintain daily assignment sheets.
• Supports and participates in the IDC process.
• Conducts regular environmental audits and completes monthly unit status reports.
• Attends monthly CNA III and related meetings.
• Performs other related duties as required.

• Knowledge of laws, rules and regulations governing assigned activities.
• Knowledge of policies, procedures, specifications, standards and guidelines governing assigned activities.
• Ability to achieve competency in taking and recording vital signs with accuracy, including parameters for reporting abnormal results to supervisors.
• Maintains CPR certification.

• Certification in dementia, hospice or other relevant discipline.
• Ability to establish effective relationships with individuals, families and team members.
• Demonstrated attention to detail with readiness to respond appropriately when needed.
• Capacity to maintain accurate records and exercise discretion in handling confidential information.

• At least two years of full‑time or equivalent part‑time professional, paraprofessional or technical experience in a hospital, clinic or medical facility providing nursing care and related services.
• At least one year of supervisory experience.
• Substitutions:
• Associate’s degree or higher in medical technology, medical assistant, nursing, nurse education or medicine may substitute up to one year of the required experience; no substitution for supervisory experience.
• Registration as a Certified Medical Assistant may substitute one year of required experience.
• Education toward such a degree will be prorated based on the proportion of the requirements actually completed.
