Overview
The Department of Developmental Services (DDS) is seeking an Autism Spectrum Disorder Human Services Coordinator I (HSC I) to serve as the primary contact for individuals and families in the Holyoke‑Chicopee Area Office. The role involves building partnerships with families, guardians, providers, and stakeholders, and coordinating services for a caseload of individuals with ASD.
Responsibilities- Coordinate development and implementation of Individual Service Plans (ISPs) for assigned clients.
- Monitor and ensure provision of services from DDS‑funded residential, day, and support program providers.
- Facilitate communication and outreach with community services and state agencies.
- Monitor safety, well‑being, and advocate for human, civil, and legal rights of clients.
- Facilitate team meetings, monitor recommendations, maintain records in relevant computer databases.
- Provide crisis intervention and plan follow‑up as needed.
- Perform related duties, including supporting Participant Directed Services.
- Familiarity with Microsoft programs, HCSIS, and Meditech.
- Experience working with children and families with intellectual disabilities.
- Skills in communication styles and/or augmentative communication systems.
Full‑time – 37.5 hours per week, 1st shift. Days off:
Saturday and Sunday. Flexibility required.
- Minimum 3 years of full‑time or equivalent part‑time professional experience in human services or social work, or equivalent combination of education and experience.
- A bachelor’s degree in a related field may substitute for 2 additional years of experience.
- Must possess a current valid motor vehicle driver’s license if required for travel.
- For certain assignments, a bachelor’s degree in social work, psychology, or a related field is required.
