What You’ll Do

As a Case Manager, you’ll be a trusted partner and advocate for consumers in our Home Care program. Your work will include:

  • Conductingthoughtful, person-centered assessments to understand each consumer’s strengths, needs, and goals
  • Developing individualized care plansthat support independence and quality of life
  • Coordinating services and community resources to ensure smooth, responsive care
  • Monitoring progress and adjusting plans to support the best possible outcomes
  • Advocatingfor consumer rights, needs, and preferences
  • Collaborating closely with asupportive, mission-driven teamand community partners
Who You Are

You’re empathetic, organized, and driven by purpose. You bring:

  • ABachelor’s degreein Social Work or a related field
  • Experience in case management, human services, or working with older adult populations
  • Knowledge of community resources and healthcare systems
  • Strong communication skills and the ability to build trust with consumers and families
  • Comfort with technology, including electronic health records
  • A valid driver’s license and reliable transportation
  • Ability to successfully complete required background checks (CORI, SORI, SAMs GSA, Mass Health Exclusion List, OIG)
