What You’ll DoAs a Case Manager, you’ll be a trusted partner and advocate for consumers in our Home Care program. Your work will include:
- Conductingthoughtful, person-centered assessments to understand each consumer’s strengths, needs, and goals
- Developing individualized care plansthat support independence and quality of life
- Coordinating services and community resources to ensure smooth, responsive care
- Monitoring progress and adjusting plans to support the best possible outcomes
- Advocatingfor consumer rights, needs, and preferences
- Collaborating closely with asupportive, mission-driven teamand community partners
You’re empathetic, organized, and driven by purpose. You bring:
- ABachelor’s degreein Social Work or a related field
- Experience in case management, human services, or working with older adult populations
- Knowledge of community resources and healthcare systems
- Strong communication skills and the ability to build trust with consumers and families
- Comfort with technology, including electronic health records
- A valid driver’s license and reliable transportation
- Ability to successfully complete required background checks (CORI, SORI, SAMs GSA, Mass Health Exclusion List, OIG)

Schedule:
After 6 months and satisfactory performance, enjoy 2 remote days and 3 in-office days each week
