Gándara Center in Hampshire County seeks a Coordinated Entry Navigator to support homeless youth ages 18-24 by providing housing-focused case management, readiness, and access to housing programs, with emphasis on education and employment, health and well-being.
You will use Positive Youth Development and Trauma-Informed Care, engage clients, connect them to community resources, complete assessments, and develop service plans, while coordinating with local partners and attending related meetings.
