We are hiring for a Case Manager RN to support patient-centered care management services within an acute care setting. This role plays a key part in coordinating care, discharge planning, resource utilization, and patient advocacy to ensure patients receive the right care at the right time and in the appropriate setting.

• Provide comprehensive inpatient care management services
• Conduct patient assessments and care planning
• Coordinate discharge planning and resource utilization
• Support regulatory compliance and documentation requirements
• Advocate for patients and families throughout the continuum of care
• Collaborate closely with interdisciplinary healthcare teams
• Ensure efficient and appropriate use of healthcare resources

• Associate Degree in Nursing required
• Active RN license
• Minimum 2 years of acute care experience in a Medical Surgical or inpatient setting
• IRR or annual competency testing in Utilization Review required
