Oasis Hospice And Palliative Care Inc. is seeking a compassionate and dedicated On Call RN to join our team in Homewood, IL. In this vital role, you'll provide expert after-hours nursing care and support to patients and families during some of life's most meaningful moments. If you're a skilled nurse with a heart for hospice and palliative care, we want to hear from you.

• Respond to after-hours patient and family calls, providing clinical assessment and guidance
• Conduct urgent and unscheduled home visits to evaluate and manage patient symptoms
• Administer medications and treatments in accordance with the patient's plan of care
• Communicate effectively with the interdisciplinary hospice team regarding patient status changes
• Provide compassionate end-of-life education and emotional support to patients and families
• Document all clinical assessments, interventions, and communications accurately and timely
• Coordinate with physicians and care team members to adjust care plans as needed

• Active RN license in the state of Illinois
• Previous hospice, palliative care, or home health nursing experience preferred
• Strong clinical assessment and critical thinking skills
• Ability to work independently and manage on-call responsibilities with professionalism
• Excellent communication and interpersonal skills with patients, families, and care teams
• Valid driver's license and reliable transportation for home visits
• Compassionate, patient-centered approach to end-of-life care
