Occupational Therapist

Occupational Therapist duties include effectively screening and evaluating patients with cognitive and/or ADL physical functioning disorders. Develop appropriate plans of care following all regulatory and clinical practice standards. Utilize standardized assessments and evidence-based practice to support clinical interventions. Provide rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.

Administer various procedures as part of the rehabilitation plan, including but not limited to ADL retraining, therapeutic activities, therapeutic exercises, modalities, and the use of supportive and assistive devices. Assess patient progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluate treatment outcomes for effectiveness. Modify services in relation to patient evaluation, and make referrals based on outcomes.

Provide consultation and counseling to patients, as well as families, caregivers, and other service providers related to physical disorders. Effectively communicate with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provide comprehensive discharge summaries of services provided per regulatory and clinical requirements. Document all regulatory and clinically required information in the electronic documentation system. Maintain patient records according to company policy.

Accurately record treatment minutes for services provided per all regulatory and clinical requirements. Participate in required meetings, including patient care conferences, utilization review meetings, and rehabilitation meetings. Engage in facility and company required training, in-services, and conferences. Obtain any necessary DME or supplies per regulatory and clinical practice requirements. Supervise and support occupational therapist assistants, rehabilitation aides, and/or students. Adhere to relevant practice acts, codes of conduct, and ethics throughout professional practice.
