As a Navian Hawaii Patient Care Manager, you will lead the after-hours team guiding patients and their family on their journey, delivering comfort and support with a caring heart, replacing fear with hope, helping them find the beauty and joy in every moment, and providing each person with the kindness and dignity they deserve.
Position Summary
The Patient Care Manager (PCM) leads an after-hours Patient Care Team based upon the shared values, mission and vision of Navian Hawaii. The PCM coordinates the activities of a multi-disciplined team to achieve the highest quality patient and family care outcomes. Rooted in evidence based practice, the PCM works closely with the Hospice Medical Directors to provide for the comprehensive physical, emotional, social and spiritual care and the individualized needs of our patients and their families.
The PCM supervises and coordinates the day-to-day delivery of patient care services and promotes the emergence of an inter-disciplinary care model, utilizing principles of hospice and palliative patient centered care. The PCM provides leadership and coaching to their staff to achieve excellence in all aspects of their practice, job responsibilities and customer service.
The PCM is accountable to the Director of Clinical Operations for implementing initiatives necessary to achieve Navian Hawaii qualitative and strategic goals.
Primary Responsibilities
Clinical Care
- Provides direct supervision to assigned clinical team members in assuring quality patient care, productivity standards and compliance with the Navian Hawaii Policies and Procedures.
- Leads the Interdisciplinary Group meetings (IDG) for the development, review and revision of the patient/family plan of care in accordance with the CMS Conditions of Participation.
- Assures accurate and timely documentation of all patient care activities in accordance with Navian Hawaii standards.
- Active member of the Navian Hawaii Quality Improvement Workgroup. Contributes to the analysis of quality data and in conjunction with the QI Workgroup, creates and implements performance improvement (PI) process to achieve quality goals.
- Performs supervisory visits with field staff.
- Thoroughly understands and works according to the Navian Hawaii values and clearly articulates the expectation of the same from his/her direct reports.
- Serves as liaison with community referral sources, and other partners in providing care, such as nursing homes, care homes, vendors, etc.
- Assists with developing expectations and the evaluation of care and services provided for Navian Hawaii patients by affiliated vendors and agencies.
- Actively develops and maintains long-term, beneficial collaborative peer relationships throughout the island of Oahu and other areas served.
- Supports the clinical experience placements professional students as approved by the DCO/President.
- Actively participates in the development and implementation of programs within Navian Hawaii in order to accomplish strategic objectives or other desired goals of the organization.
- Evaluates staffing needs and recommends changes to the DCO,
- Identifies learning needs of direct reports and works in conjunction with the DCO and others to promote improved clinical care delivery. Coordinates staff education and clinical mentoring to achieve individualized personnel development goals.
- Provides timely coaching and feedback to clinical staff to achieve performance goals.
- Participates in recruitment and professional development of clinical staff. Coordinates and implements orientation plan for new personnel [with Human Resources]
- Contributes to the development of in-service education for all the Navian Hawaii staff and volunteers as requested
- Actively engages in public and professional education, directly through Navian Hawaii's efforts and/ or in collaboration with others in the community. Coordinates such activity with DCO and Director of Marketing and Business Development.
- Participates in performance evaluation and disciplinary action (if needed) of staff.
- Participates in developing and revising written patient care policies and procedures.
- Serves as a member of the Navian Hawaii Clinical Management Team.
- Provides cross coverage for other members of the Clinical Management Team
- Responsible for supporting compliance with licensing and Medicare certification, including all rules and regulations from Chapter 94 and CMS, as applicable.
- Manages daily personnel administration tasks for direct reports, e.g. PTO requests, coverage for leaves, etc.
- Enter rates and evaluates audit results.
