What You'll Do

* Provide in-home speech therapy services for home health patients

* Perform evaluations and develop individualized treatment plans

* Address swallowing disorders, cognitive-communication deficits, speech/language impairments

* Collaborate with nurses, therapists, and case managers

* Maintain accurate documentation in a Medicare home health setting

What We're Looking For

* Current Hawaii license as a Speech-Language Pathologist (SLP)

* 1+ year clinical experience (home health experience a plus)

* Strong documentation and communication skills

* Ability to work independently in the field

* Valid driver's license + reliable transportation

* Heart for serving seniors and local families
