Physical Therapist (PT) – Home Health Care
Pay: $80 per visit
Location: O'ahu, HI
Schedule: Flexible PRN Schedule

We are looking for a Physical Therapist who is passionate about patient-centered care and enjoys working one‑on‑one with patients in a home setting.

What You'll Do- Conduct physical therapy evaluations and reassessments
- Create individualized treatment plans
- Address mobility, balance, strength, and safety concerns
- Provide patient and caregiver education
- Monitor progress and adjust treatment plans as appropriate
- Coordinate care with the interdisciplinary team
- Complete Medicare‑compliant documentation
- Hawaii Physical Therapist license
- Minimum one year of clinical experience
- Home health experience preferred
- Reliable transportation
- Strong communication and organizational skills
- Flexible caseload options
