Home Health Physical Therapist (PT) | Oʻahu | $80–$85/Visit

Always Best Care is a Medicare-certified home health agency serving communities across Oʻahu. Our mission is to help seniors remain safe, independent, and comfortable in their homes by providing in-home health services focused on improving mobility, reducing fall risk, and enhancing quality of life. We are seeking a compassionate, licensed Physical Therapist (PT) to join our clinical team with flexibility and autonomy.

• Provide skilled physical therapy services in patients  homes.
• Perform comprehensive initial evaluations, reassessments, and fall risk assessments.
• Develop individualized treatment plans to improve strength, mobility, balance, and functional independence.
• Educate patients and caregivers on home exercise programs and injury prevention.
• Collaborate with nurses, physicians, and the interdisciplinary care team to ensure quality patient outcomes.
• Complete accurate, timely, and compliant clinical documentation.

• Current Hawaiʻi Physical Therapist (PT) license in good standing.
• Minimum of one year of clinical experience preferred.
• Home Health experience is a plus, but not required.
• Strong clinical judgment, communication, and documentation skills.
• Valid driver s license, reliable vehicle, and current auto insurance.
• Ability to travel throughout Oʻahu to provide in-home patient care.
