Compensation: $80 per visit

Our mission centers on empowering seniors to remain safe, independent, and thriving in the comfort of their own homes through targeted fall prevention, mobility enhancement, and strength building.
We are seeking a licensed Physical Therapist (PT) who is passionate about delivering high-quality, patient-centered care directly to local families and kupuna.
Responsibilities- Deliver skilled, compassionate in-home physical therapy services.
- Conduct comprehensive initial evaluations, ongoing reassessments, and detailed fall risk assessments.
- Design and execute individualized treatment plans focused on balance, strength, and spatial safety.
- Empower patients and their multi-generational caregivers through tailored home exercise program education.
- Ensure accurate, compliant clinical documentation and collaborate smoothly with our interdisciplinary care team.
- Active, Hawaii license as a Physical Therapist (PT).
- Minimum of one year of clinical experience (home health experience is a huge plus, but we welcome strong clinical backgrounds).
- Excellent communication, time management, and independent clinical documentation skills.
- Valid driver’s license and reliable transportation to travel to patient homes.
