Physical Therapist (PT) – Home Health Care
Pay: $80 per visit
Location:
O'ahu, HI (reliable transportation required)
Schedule:

Always Best Care is seeking a licensed Physical Therapist to provide skilled home health services throughout O'ahu. You will evaluate patients, develop treatment plans, and provide therapy interventions designed to improve mobility, safety, strength, and overall function.
Responsibilities- Provide skilled physical therapy services in patients' homes
- Complete evaluations, reassessments, and fall risk assessments
- Develop individualized treatment plans
- Improve patient mobility, balance, strength, and safety
- Educate patients and caregivers on exercises and fall prevention
- Communicate patient progress with the interdisciplinary care team
- Maintain Medicare-compliant documentation
- Current Hawaii Physical Therapist license
- Minimum one year of clinical experience
- Home health experience preferred
- Strong communication and documentation skills
- Ability to work independently
- Valid driver's license and reliable transportation
