Physical Therapist

Provide specialized physical therapy services to patients to improve mobility, strength, and overall function. Collaborate with a multidisciplinary team to develop and implement personalized treatment plans that support patient recovery and discharge goals.

Responsibilities:

• Assess patients’ physical condition and identify therapy needs through comprehensive evaluations.
• Develop and adjust individualized treatment plans based on patient progress and goals.
• Deliver therapy interventions utilizing appropriate clinical techniques and therapeutic protocols.
• Document clinical findings, treatment progress, and discharge plans accurately within established deadlines.
• Supervise and mentor assigned therapy assistants to ensure quality care delivery.
• Communicate care plans and progress updates effectively with patients, families, and team members.

Qualifications:

• Hold a valid Hawaii state Physical Therapy license.
• Possess at least 2 years of outpatient or healthcare setting PT experience.
• Demonstrate proficiency with Epic electronic health record systems.
• Have a Bachelor's degree in Physical Therapy (Master's degree preferred).
• Exhibit at least 1 year of home health therapy experience.
• Be willing to travel as needed for patient care.
