Key Responsibilities- Conduct psychosocial assessments, develop and implement social work service plans, and evaluate practice effectiveness.
- Perform case management, coordinate services, and provide resource identification, referral, and advocacy.
- Assist with program development, policy implementation, and quality improvement initiatives.
- Participate in research, documentation, departmental meetings, and staff training.
- Masters degree in social work and licensure as a Licensed Clinical Social Worker or Licensed Social Worker.
- Basic Life Support certification from the American Heart Association.
- Preferred: at least one year of social work experience, healthcare experience with medical diagnoses, and strong networking skills with community agencies.
