Physical Therapist and Physical Therapist Assistant - Home Health

Pay: PT: $80 per visit; PTA: $55-$60 per visit.

Location:

O’ahu, HI (reliable transportation required).

Always Best Care Home Health provides skilled services to patients across O’ahu. Our mission is to help seniors and kupuna improve mobility, reduce fall risks, and maintain independence while receiving care in the comfort of their homes.

• Provide skilled in‑home physical therapy services.
• Complete evaluations, reassessments, and fall risk assessments (PT).
• Implement individualized treatment plans to improve mobility, balance, strength, and safety.
• Educate patients and caregivers on home exercise programs and fall prevention.
• Document accurately and communicate patient progress with the interdisciplinary care team.
• Maintain compliance with Medicare home health documentation requirements.

• Current Hawaii license as a Physical Therapist (PT) or Physical Therapist Assistant (PTA).
• Minimum one year of clinical experience (home health experience preferred).
• Strong documentation, communication, and time‑management skills.
• Ability to work independently in the field.
• Valid driver’s license and reliable transportation.
