Aloha Care in Honolulu is seeking a D-SNP Care Navigator to assist enrolled members with understanding benefits and accessing services. The role is largely telephonic, coordinating with providers and the interdisciplinary care team to ensure timely care and appropriate plans.
You will conduct initial welcome calls, health risk assessments, and develop Care Plans, coordinating referrals, authorizations, and social resources while maintaining compliance and accurate documentation.
