POSITION SUMMARY

The Clinical Case Manager II provides one-on-one and group clinical interventions to clients assigned to the Stanislaus County Juvenile Program in-custody unit. Work hours are Monday-Friday 8:30am-5:00pm.

TYPICAL RESPONSIBILITIES AND DUTIES

The Clinical Case Manager II will review case plans, provide therapeutic inventions, and ensure follow through on linkages to mental health, substance abuse treatment, and other services and provide support.

ESSENTIAL FUNCTIONS
  • Participate in the intake of participants as required, conduct initial interviews of participants assigned to the program, and provide clinically informed feedback to program staff.
  • Maintain confidentiality of information obtained through job duties regarding employees, vendors, participants, outside agencies, etc. so that sensitive information is only furnished on ‘need to know’ basis.
  • Familiarity with the LCA Employee Handbook, Personnel Policy and Operations Manual, and Operating Procedures Manual (all procedures and policies included on the LCA forms and files site).
  • Develop an individualized therapeutic alliance with clients in the program.
  • Serve as primary liaison between clients and their family members, custody staff, support team, and any other ancillary treatment team members.
  • Complete all relevant required documentation, including treatment plans, progress notes, and any other ancillary treatment team members.
  • Make recommendations regarding caseload matters.
  • Conduct and participate in case conferences for each participant assigned to the program.
  • Provide updates of participant status at case conferences.
  • Track participant participation in all groups and programs in the database system.
  • Coordinate and communicate with all other program staff and outside referrals to ensure consistency and fidelity to the program’s mission.
  • Attend staff meetings as is feasible.
  • Observe and notify pertinent staff of significant participant behaviors, e.g. changed presentation, interactions, participation in groups, or other notable participant conduct.
  • Immediately notify supervisory staff in the event of a potentially dangerous situation.
SPECIALIZED FUNCTIONS
  • Lead therapeutic rehabilitative program groups.
  • Conduct individual and group counseling sessions with a focus on CBT and Mindfulness interventions.
  • Assist participants in integrating clinical and group work into everyday life.
  • Participate and assist Program Director/Director of Program Services in conducting internal client records audits and quality assurance reviews.
  • Notify the Reentry Program Director/DDS and the relevant staff of any clinical and mental health concern involving any program or participant services/treatment plans.
  • Follow a curriculum that exercises psychotherapeutic interventions to ensure fidelity of treatment.
EDUCATION/WORK EXPERIENCE REQUIREMENTS
  • Master’s degree (MSW / MFT preferred).
  • Active clinical license strongly preferred but not required.
  • Preference for candidates with familiarity and/or experience working in collaboration with juvenile justice/probation/individuals involved with the criminal justice system.
  • Must be eligible or have an active NPI Number.
  • Must have previous experience documenting in electronic health records systems, and demonstrate efficiency in utilizing such systems.
  • Must have an active CPR/First-Aid Certification (if not, will be required to attain within 90-days after hire).
  • Strong preference for bilingual candidates;
    English and additional language(s).
QUALIFICATIONS
  • Must have a valid California Driver's License with a clean driving record and proof of insurance coverage that meets company vehicle insurance standards.
  • Safe and reliable vehicle.
  • Ability to read, write, and speak the English language. Ability to communicate in a second language, such as Spanish, highly preferred.
  • Strong written and verbal communication skills.
  • Ability to express thoughts logically, in written and verbal form using correct grammar.
  • Ability to multi-task to meet program objectives.
  • Ability to maintain confidential information obtained through job duties regarding clients, employees, vendors, outside agencies, etc.
