On-Call Social Worker

This position provides social work services including assessment, care planning, case management, and coordination for patients and their families. It involves developing and evaluating social work plans, offering psychosocial assessments, and delivering individual, family, and group interventions to address emotional, mental, and behavioral health issues.

The role also includes acting as a liaison between healthcare services and the community, facilitating resource referrals, advocacy, and service coordination to ensure appropriate patient placement. Responsibilities extend to program development, policy implementation, quality improvement, research, and maintaining documentation in compliance with regulations.

• Master’s degree in social work with licensure as a Licensed Social Worker or Licensed Clinical Social Worker in Hawaii.
• Basic Life Support certification from the American Heart Association.
• Experience working with medical diagnoses and networking with community agencies.
• Preferred: at least one year of social work experience.

• Perform assessments, develop and implement social work care plans, and evaluate practice effectiveness.
• Coordinate case management, provide resource referrals, and advocate for patients.
• Participate in program development, quality improvement, and departmental activities.
• Conduct research, document services, and ensure compliance with regulations.
