About This Role
The Coordinator Patient Care-Case Management role involves coordinating with physicians and staff to develop and monitor patient care plans and discharge plans. This position is ideal for individuals who are skilled in clinical activities and have a strong understanding of healthcare regulations.

- Schedule: Full Time
- Perform daily pre-admission, admission, and concurrent utilization reviews.
- Escalate utilization and system problems that are unresolved.
- Conduct rounds and review patient admissions under Kaiser Skilled Nursing Facility benefit.
- Utilize knowledge of Medicare regulations to determine medical necessity.
- Monitor the progression of the plan of care and facilitate discussions with multidisciplinary teams.
- Educate healthcare team members on utilization and cost containment initiatives.
- Collaborate with patients, families, physicians, and staff regarding care provisions.
- Develop, evaluate, and coordinate comprehensive discharge plans.
- Monitor care processes and assist in developing guidelines for patient care.
- Identify opportunities for cost-savings and improvements in quality of care.
