Licensed Practical Nurse (LPN) — Per Diem (Home Health)
Pay: $60 per visit
Location:
Oahu, Hawaii (must have reliable transportation — we serve the entire island)
Schedule:
Flexible / Per diem — build your own caseload
We are a growing, Honolulu‑based Medicare‑certified Home Health agency serving patients across all of Oahu. We focus on providing high‑quality care that helps local seniors remain safe, supported, and independent at home.

Responsibilities- Provide skilled nursing visits in patients’ homes under RN direction
- Assist with medication management and patient education
- Perform wound care and routine skilled nursing tasks within scope
- Communicate changes in patient condition to RN / clinical team
- Document accurately and on time
- Current Hawaii LPN license li>1+ year clinical experience (home health preferred)
- Strong communication and documentation skills
- Valid driver’s license + reliable transportation
- Ability to work independently in the field
