Compensation: Starting at $105,000–$110,000 annually

• Coordinate and manage the admissions process from referral through admission
• Conduct clinical reviews and assessments to support admission decisions
• Collaborate with hospitals, physicians, case managers, residents, and families
• Ensure all admission documentation is completed accurately and in compliance with regulatory requirements
• Facilitate smooth transitions into the facility while providing exceptional customer service
• Serve as a resource for residents, families, referral partners, and members of the care team
• Help drive census growth through strong relationship management and responsiveness

• Current, active Registered Nurse (RN) license in the State of Hawaii
• Strong clinical assessment and critical thinking skills
• Ability to make sound independent decisions

• Knowledge of nursing practices, medical procedures, and long-term care regulations
• Excellent communication, customer service, and interpersonal skills
• Ability to work effectively with an interdisciplinary team
• Strong organizational, analytical, and problem-solving abilities
• Positive attitude and genuine passion for serving older adults
• Recent geriatric nursing experience required
• Long-term care experience preferred
