Registered Nurse (RN) Long-Term Care (LTC) Position Overview
We are seeking an experienced Registered Nurse (RN) to join a Long-Term Care (LTC) team on a contract assignment. The RN will provide comprehensive nursing care to residents, coordinate care plans, administer medications and treatments, and collaborate with interdisciplinary teams to ensure exceptional resident outcomes. The ideal candidate is compassionate, clinically skilled, and experienced in caring for geriatric and long-term care populations.
Key Responsibilities- Deliver high-quality nursing care to residents in a long-term care setting.
- Perform comprehensive resident assessments and develop individualized care plans.
- Administer medications, treatments, and therapeutic interventions in accordance with physician orders and nursing standards.
- Monitor resident conditions and promptly report significant changes to the healthcare team.
- Coordinate and collaborate with physicians, therapists, social workers, and other interdisciplinary team members.
- Maintain accurate and timely documentation in the electronic medical record (EMR) system.
- Supervise and support Certified Nursing Assistants (CNAs) and other support staff as appropriate.
- Ensure compliance with infection control standards, patient safety protocols, and regulatory requirements.
- Participate in care conferences, quality improvement initiatives, and resident education activities.
- Provide compassionate support and education to residents and their families.
- Active Registered Nurse (RN) license in good standing.
- Minimum of five (5) years of nursing experience.
- Recent Long-Term Care (LTC), Skilled Nursing Facility (SNF), rehabilitation, or geriatric nursing experience.
- Strong clinical assessment and critical thinking skills.
- Excellent communication, interpersonal, and organizational abilities.
- Ability to work independently and collaboratively within a multidisciplinary healthcare team.
- Basic Life Support (BLS)
- Advanced Cardiovascular Life Support (ACLS)
- Additional specialty certifications relevant to long-term care nursing
- Rotating Shifts
- 40 Hours per Week
- Weekend Coverage Required as Needed
- Holiday Coverage Required
- Contract Position
- 13-Week Assignment
- Experience with electronic medical record (EMR) systems preferred.
- Familiarity with Point Click Care or similar long-term care EMR platforms is advantageous.
- Manage resident care across varying levels of acuity within the long-term care setting.
- Evaluate resident responses to treatment and modify care plans as appropriate.
- Respond effectively to emergencies and changes in resident conditions.
- Ensure proper documentation and communication regarding resident care and outcomes.
- Active state RN licensure must be obtained prior to the start of the assignment.
- Candidates must be available to work rotating schedules, including weekends and holidays as required.
- References must be submitted according to assignment requirements.
