Overview
The Psychiatric RN Coordinator coordinates, monitors, and ensures that appropriate and timely mental health, acute and long‑term care services are provided to members across the continuum of care. The role serves as a key member of the Interdisciplinary Care Team (ICT), leading care coordination on all islands and collaborating with CCS management, psychiatrists, and community partners.
Primary Duties and Responsibilities- Conduct face‑to‑face visits with CCS members for assessments, teaching, reviewing, and administering ordered medications.
- Develop and provide training to agencies, community partners, and staff regarding behavioral health and medical health integration and clinical care.
- Order and administer medication as directed by the CCS psychiatrist and coordinate the ordering and administering of medications by other RN case managers.
- Interact with members, families, physicians, and other providers using clinical knowledge to determine medical history and current status, and to assess care options, including benefits and community resources, to update the Care Plan.
- Coordinate community resources and manage benefits, regulations, laws, and public entitlement programs.
- Maintain HIPAA standards and confidentiality of protected health information; report critical incidents and quality‑of‑care issues.
- Utilize member electronic records data to verify that services meet members’ needs.
- Facilitate member and provider authorization and access to services.
- Resolve concerns about care delivery or providers.
- Develop and provide education to staff, community partners, agencies, and patients on medical conditions such as hypertension, diabetes, and wound care.
- Provide education on medication use and conduct community educational sessions for people with SMI, families, and partners.
- Participate in IDT meetings.
- Act as liaison and advocate between members/families, physicians, and facilities/agencies.
- Maintain accurate case management records in the system using clinical guidelines.
- Ensure compliance with all state and federal regulations and guidelines.
- Perform all other duties assigned.
- Adhere to regulatory compliance, quality guidelines, and Aloha Care policies and procedures.
- Maintain confidentiality of Aloha Care information per policies, state and federal laws.
- Work evenings and weekends as needed.
- Work in a fast‑paced, changing environment, managing multiple projects across teams in a matrixed structure.
- Perform required prolonged sitting and computer workstation operation.
- Minimum of 5 years of healthcare experience with client care coordination responsibilities in behavioral health.
- Minimum of 1 year of experience working with a severely mentally ill (SMI) population.
- Licensed professional in current case management for a minimum of 1 year.
- Experience in a managed care environment.
- Excellent time management, organization, and communication skills; dependable, self‑starting, and self‑motivated.
- Proficient in data entry with high accuracy and attention to detail.
- Ability to work independently with minimal supervision.
- Exceptional performance under pressure.

CompensationSalary Range: $65,000 - $83,000 annually
