Experience:
1 year of Home Health experienceMust have 6 months to 1 year+ of Oasis documentation experienceType: Full-time;
Contract
Schedule: Monday - Friday, 8:00am to 4:30pm (No on-call/weekends)

Job Description:
Conduct comprehensive patient assessments and manage all phases of home health care, including Admissions, Start of Care (SOC), Routine Visits, Recertifications, Resumption of Care (ROC), Transfers, Discharges, and Significant Change in Condition evaluationsProvide ongoing case management and coordinate patient care plans to ensure quality outcomes and continuity of careMaintain accurate, timely, and compliant clinical documentation in accordance with home health documentation guidelines and regulatory standardsAssess patient progress, identify changes in condition, and communicate updates to physicians and interdisciplinary care teamsManage discharge planning, including discharges related to safety concerns or patient non-complianceDeliver skilled nursing care across a variety of home health and patient care settings while promoting patient safety and independencePosition Requirements:
