Home Health Registered Nurse

Location:

Oahu, Hawaii

Assignment Duration:
This is a 13-week contract position.

Schedule:

Days, 8 hours per shift, 40 hours guaranteed weekly.

Job Description:

The Home Health Registered Nurse (RN) plays a crucial role in providing excellent patient care through the collaboration, development, implementation, revision, and reporting of the case management program. The RN serves as a liaison among the patient, family, nurse, physicians, multidisciplinary team, and healthcare benefactor to optimize outcomes and enhance the quality of care.

Responsibilities:

  • Lead the coordination of patient care with other disciplines within the care team.
  • Monitor diagnostic testing, treatments, and procedures, ensuring appropriateness and timeliness of care.
  • Discuss with physicians the appropriateness of resource utilization, treatment plans, and discharge planning.
  • Focus on complex patient needs including chronic pain, substance abuse, and homelessness.
  • Collaborate with acute care case managers for appropriate ongoing care.
  • Coordinate transfers to tertiary centers, ensuring continuity of care with all necessary patient information.
  • Collect and oversee data for regulatory and accreditation compliance.
  • Manage frequent emergency department visitors, conducting focus study reviews of previous admissions.

Required Experience / Certifications / Licensure:

  • 2+ years of nursing experience (required).
  • 6 months of home health experience (required).
  • Travel experience (required).
  • Hawaii RN license (required).
  • Current driver's license and car insurance (required).
  • Experience with Homecare Home Base and OASIS (required).
  • COVID and Flu vaccines (required).
  • Bachelor's degree (preferred).
  • Case management experience (preferred).
