Kahala Nui, considered Hawaii's premier Senior Living Community is looking for a full time MDS Coordinator to join our team. The MDS Coordinator is responsible for coordinating and completing the Resident Assessment Instrument (RAI) process, including MDS assessments, care area assessments, and care plans for residents in the Nursing department. This role ensures compliance with federal and state regulations, supports accurate reimbursement through proper documentation and coding, and collaborates with interdisciplinary team members to promote quality resident care.

This position in 100% on-site at Kahala Nui. Remote or hybrid remote work is not available.

Key Responsibilities
  • Coordinate and complete MDS assessments in accordance with CMS guidelines and required timelines.
  • Ensure accurate coding and documentation to support regulatory compliance and reimbursement.
  • Develop, review, and update individualized resident care plans in collaboration with the interdisciplinary team.
  • Monitor resident assessment schedules and submit MDS data electronically as required.
  • Conduct chart audits and documentation reviews to ensure accuracy and compliance.
  • Educate nursing staff and other team members on MDS requirements and documentation standards.
  • Participate in quality assurance and performance improvement initiatives.
  • Communicate with physicians, residents, families, and healthcare team members regarding resident care needs and assessment findings.
Minimum Qualifications
  • Current Hawaii RN license in good standing.
  • Experience in long-term care, skilled nursing, or rehabilitation services.
  • Knowledge of MDS 3.0, RAI process, Medicare, Medicaid, and CMS regulations.
  • Strong assessment, analytical, organizational, and communication skills.
  • Proficiency with electronic health records (EHR) and MDS software systems.
