About This Role
The Coordinator Patient Care-Case Management position involves coordinating with healthcare providers to develop and monitor patient care plans, ensuring continuity of care and compliance with regulations. - Coordinate with physicians, staff, and non-Kaiser providers regarding patient care.
- Develop plans of care and discharge plans in conjunction with physicians.
- Perform daily pre-admission, admission, and concurrent utilization reviews.
- Monitor the progression of the plan of care and facilitate discussions with multidisciplinary teams.
- Educate healthcare team members on utilization and cost containment initiatives.
- Collaborate with patients, families, and staff regarding care provisions.
- Develop, evaluate, and coordinate comprehensive discharge plans.
- Monitor care processes and assist in developing guidelines and care paths.
- Identify opportunities for cost-savings and improvements in quality of care.
- Provide direct patient care as needed to maintain clinical expertise.
