LSW Inpatient​/Social Services

Kaiser Permanente seeks an LSW Inpatient/Social Services professional in Honolulu to provide compassionate, tech-enabled inpatient social work support. In this role, you will complete psychosocial assessments, lead discharge planning, and coordinate care with physicians, nurses, and behavioral health teams. You will counsel patients and families, connect them with community resources, and advocate for their needs. Using modern digital tools and EHR systems, you will document care, track outcomes, support crisis interventions, and collaborate in a culture focused on innovation and continual improvement.

Responsibilities

• Provide psychosocial assessments and discharge planning for inpatient clients
• Coordinate care plans with medical, nursing, and behavioral health teams
• Facilitate patient and family counseling, education, and resource referral
• Advocate for patients' needs and connect them with community services
• Maintain accurate documentation and comply with regulatory standards
• Collaborate with cross-functional teams to improve care workflows
• Use digital tools and software to track cases and outcomes
• Participate in case conferences and multidisciplinary team meetings
• Support crisis intervention and safety planning when needed
• Contribute feedback to enhance internal systems and patient experience
