Social Work Program Coordinator

The Social Work Program Coordinator is administratively responsible for the provision of social work services and case management in the Polytrauma/Traumatic Brain Injury (TBI) and Spinal Cord Injury (SCI) Programs within Physical Medicine and Rehabilitation (PMR) Service at the VA Pacific Islands Health Care System (VA PIHCS). Major duties include but are not limited to the following:

• Provides ongoing comprehensive social work services to Veterans with spinal cord injuries and traumatic brain injuries through the coordination of health and rehabilitation services and addressing the psychosocial needs of Veterans and their families.
• Manages aspects of program operations related to the provision of social work services by providing direct patient care, developing relevant policies and procedures for program operation, and preparing reports and statistics for facility, VISN, and national use.
• Delivers services at the advanced practice level and has specialized knowledge of practice related to case management and psychosocial impacts of spinal cord injury, polytrauma, and traumatic brain injury.

Administrative Functions:

• Coordinates, develops, evaluates, and delivers social work services in the TBI and SCI Programs in PMR Service.
• Develops successful collaborations and ongoing consultation with the PMR multi-disciplinary teams, CBOCs, Primary Care, Social Work Executive, and other VA staff and community-based programs to ensure quality to support Veterans' psychosocial needs.
• Ensures compliance with the regulations and requirements which govern provision of social work services in the SCI and TBI programs.
• Administers information and analytical systems to evaluate and enhance the quality of service provided to patients, maintains related local and national databases, and monitors related performance measures.

Clinical Functions:

• Provides advanced level case management and social work interventions for Veterans with traumatic brain injuries, other acquired brain injuries, and spinal cord injuries and is accountable for the provision and effectiveness of case management and family support services provided in SCI and TBI Programs.
• Independently interviews Veterans and families to assess presenting problems and their impact on functioning and health, utilizing professional skill, objectivity, and insight during assessments and interviews to determine viable treatment options and resources.
• Coordinates treatment plans with interdisciplinary teams, assists with community resources, and acts as liaison with service providers to maintain or improve veteran's health and social functioning in a supportive environment.
• Assists Veterans and families with advance directives, guardian ships, and applications for home care and extended care services.

Work Schedule:

Full-Time;
Monday to Friday, 7:30am-4:00pm

Pay:
Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.

Parental Leave:
After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Child Care Subsidy:
After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66

Retirement:
Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA

Insurance:
Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)

Telework:
Ad-Hoc Telework may be authorized at Supervisory discretion.
