School Based Health Center (SBHC) Medical Provider

The Hood River Valley (HRV) High School Based Health Center (SBHC) Medical Provider (PA or NP) will be responsible for delivering direct patient care across all age groups within the scope of credentials and privileges granted, following a PCPCH (Patient-Centered Primary Care Home) care model at the HRV School Based Health Center (SBHC).

Responsibilities include:

  • Conducting patient assessments
  • Diagnosing conditions
  • Providing treatment for diagnosed conditions
  • Prescribing medications
  • Performing specialized tests and treatments

Schedule:

  • 16–24 hours/week (two to three 8:00 AM–5:00 PM shifts - 8 hour shift)
  • Additional hours may be available up to 1.0 FTE at our main Hood River clinic

Minimum Qualifications:

  • Licensed Family Practice Physician Assistant or Nurse Practitioner in good standing in Oregon
  • Current DEA certification
  • Current Basic Life Support (BLS) certification
  • Minimum of three (3) years of professional clinical experience in family or general practice, including prenatal, adult, and pediatric primary care (acute and chronic conditions); experience in office gynecology or emergency care is a plus but not required
  • Maintenance of required Continuing Education (CE) credits
  • Graduation from an accredited PA or NP program with current board certification (NCCPA for PAs, specialty board for NPs)
  • Excellent oral and written communication skills, including public speaking
  • Ability to communicate clinical concepts effectively to clinical and non-clinical audiences
  • Proficiency in collecting, analyzing, and presenting statistical data and reports in written form
  • Capacity to work efficiently within a diverse, multicultural workforce
  • Demonstrated ability to deliver care to culturally diverse populations with compassion and sensitivity
  • High-level computer skills, including Microsoft Office (Word, Excel, Outlook), Electronic Health Record (EHR), and Electronic Practice Management (EPM) systems
  • Strong organizational, prioritization, and coordination skills to manage multiple activities and team members effectively
  • Capability to delegate tasks and collaborate with team members
  • Ability to remain calm and effective in high-pressure and emergency situations
  • Exceptional interpersonal, analytical, and problem-solving skills
  • Capacity to make independent decisions in accordance with established best practices and protocols
  • Professional leadership qualities, with the ability to represent the organization positively within the community
  • Proficient primary care clinical skills
  • Collaborative work ethic within the PCPCH team and broader organizational framework
  • Ability to work with high initiative, energy, and effectiveness in a fast-paced environment
  • Accessible phone service and reliable transportation for after-hours call rotation, if applicable
  • Thorough knowledge of HIPAA regulations and public health principles
  • Adherence to the standards of nursing ethics and professional conduct as outlined by state statutes and licensing boards in Oregon and Washington
  • Successful relationship management skills with multidisciplinary teams in a diverse, multicultural environment
  • Effectiveness in working within an operation that promotes teamwork and handles ambiguity
  • Capability to work independently, manage multiple projects, and maintain professionalism and confidentiality

Preferred Qualifications:

  • Experience in a community or migrant health center (FQHC) or public health clinic setting
  • Knowledge and/or experience in team leadership (preferred)
  • Experience with EPIC EMR

Compensation:

  • Salaried/Exempt:
    Starting $130k, increases based on experience and licensure
  • Bilingual Differential:
    Additionally, we offer $0.75 per hour as compensation for bilingual proficiency in both English and Spanish.
