Providence Health & Services is seeking an LPN/LVN for a Hospice Home Health role serving the Columbia Gorge service area. The position travels to patients’ homes, with a weekday day shift and rotating weekend coverage.
Under RN supervision you will provide nursing care, collect assessment data, contribute to care planning, and implement the plan of care within licensure. Driving is required to visit patients in Oregon and Washington; a valid license and auto insurance are mandatory.
