We're seeking a detail-oriented RN MDS Coordinator to join our clinical leadership team.
This position allows you to move beyond traditional bedside nursing while continuing to make a direct impact on resident care. You'll use your nursing expertise, critical thinking, organization, and documentation skills to help ensure each resident's needs and services are accurately reflected throughout the assessment and care-planning process.

Position OverviewThe RN MDS Coordinator is responsible for coordinating the resident assessment process and helping ensure accurate, timely completion of the Minimum Data Set (MDS) and related documentation.
You'll work closely with the interdisciplinary team to coordinate assessments, support individualized care planning, monitor documentation, and promote compliance with applicable requirements and facility standards.
Key Responsibilities- Coordinate and complete MDS assessments accurately and within required time frames
- Review clinical documentation to ensure resident conditions, services, and care needs are accurately represented
- Coordinate assessment schedules and interdisciplinary participation
- Collaborate with nursing, therapy, dietary, social services, physicians, and other members of the care team
- Participate in the development and review of individualized resident care plans
- Monitor documentation for completeness, consistency, and accuracy
- Identify documentation concerns and coordinate appropriate clarification or follow-up
- Support accurate reimbursement through thorough clinical assessment and documentation
- Monitor resident changes that may require additional assessment or care-plan review
- Participate in interdisciplinary meetings and care conferences
- Assist with audits, quality improvement initiatives, and survey readiness
- Serve as a resource to nursing staff regarding MDS documentation and assessment requirements
- Maintain confidentiality of resident and clinical information
- Current Registered Nurse (RN) license in good standing
- Strong clinical assessment and critical-thinking skills
- Excellent organization and attention to detail
- Strong written and verbal communication skills
- Ability to manage multiple deadlines and priorities
- Proficiency with electronic clinical documentation systems
- Ability to work effectively with an interdisciplinary healthcare team
- Previous MDS Coordinator or MDS experience
- Skilled nursing, rehabilitation, or long-term care experience
- Knowledge of resident assessment, care planning, and reimbursement processes
- Experience with electronic medical records and MDS software
- RAC-CT or other applicable MDS-related certification is a plus
