Position Overview
Provides comprehensive clinical care to patients, coordinating with interdisciplinary teams to deliver high‑quality, evidence‑based outcomes. Position is full‑time with a weekly schedule of 40 hours and a pay range of $ to $.
Responsibilities- Perform physical assessment of patients to identify priority care needs.
- Document patient assessment and teaching and learning needs.
- Obtain detailed nursing history and identify patients at physiologic and psychological risk.
- Consult with interdisciplinary team regarding assessment findings.
- Perform procedures necessary to implement plan of care based on care standards and protocols.
- Develop plan of care across the continuum and exercise discretion and judgment when revising based on physiologic and psychosocial findings.
- Order tests and consults as required by plan of care and review test results.
- Review patient plan of care with interdisciplinary team.
- Discuss plan with patient and family.
- Provide safe therapeutic care in a holistic and systematic way to facilitate patient outcomes.
- Demonstrate ability to review and evaluate plan of care for effectiveness of desired outcomes, revising initial plans as necessary.
- Document all patient care activities per documentation standards.
- Evaluate outcomes of documentation and contribute to performance improvement initiatives through medical record review.
- Show effective communication skills and assist in conflict resolution among health care team members, patients, and families.
- Collaborate with interdisciplinary team and incorporate expertise to achieve patient outcomes.
- Serve as a change agent by implementing practice initiatives.
- Coordinate patient care activities to maximize outcomes.
- Facilitate assessment of patient learning needs and outcome measures of education.
- Access resources from all disciplines to achieve patient outcomes.
- Facilitate communication among departments for continuity of care.
- Act as primary resource to patient care team.
- Identify, provide, develop, and maintain learning resources for team.
- Lead quality improvement processes and recognize trends in patient populations, exploring alternatives to care and revising standards, guidelines, and protocols.
- Analyze practices that contribute to variance, draw conclusions from aggregate data, and identify resources to assist in variance analysis.
- Assist in development and revision of standards for patient populations incorporating interdisciplinary approach and best practices.
- Develop and revise patient education programs and evaluate effectiveness.
- Participate in clinical research, review current nursing literature, and apply findings to practice.
- Recognize behaviors impacting team function and implement strategies to optimize teamwork; serve as mentor for team.
- Seek leadership opportunities and facilitate leadership development among colleagues.
- Collaborate in development and implementation of educational plan for identified needs.
- Review self‑development plan and professional goals; seek new knowledge toward professional growth.
- Demonstrate evidence of self‑development by participating in educational programs.
- Provide educational programs to advance staff performance.
- Physician Assistant: Graduate of an accredited physician assistant program approved by the National Commission of Certification of Physician Assistants (NCCPA); 1+ year experience in primary or acute care; NJ PA license.
- Registered Nurse – Nurse Practitioner: Bachelor’s degree in nursing plus Master’s in Advanced Clinical Practice or post‑Master certificate for nurse practitioner; 1+ year experience in primary or acute care; NJ NP license.
- Advanced Practice Nurse: NJ license (any advanced practice nursing designation).
