Supports the admission, discharge, and transfer process to facilitate patient throughput in the organization. Consistently maintains open communication to assist in the admission and discharge needs of the organization. Serves to ensure a safe and efficient admission and discharge process for patients through collaboration with the healthcare team, patient, and patient support system; use of teach back methodology, and referrals to external community services as appropriate.
Works on various units throughout the shift based on the admission and discharge volumes as an integral member of the healthcare team.
- Graduate of an accredited school of nursing. Bachelor of Science in Nursing (BSN) preferred.
- Four years of experience in an acute care setting. Previous telemetry experience preferred.
- AHA BLS – Healthcare Provider, Registered Nurse – NJ.
- Strong verbal and written communication skills.
- Excellent customer service orientation, conflict resolution, and negotiation skills.
- Critical thinking, logic, and reasoning skills to address complex situations.
- Demonstrates responsiveness to the needs of patients, other care team members, and self.
- Organizes and prioritizes in a fast‑paced environment and leverages interdepartmental relationships to remove obstacles.
- Leans on hospital‑based programs for lab results, bed tracking, and other areas and can abstract medical information.
- Possesses computer skills, including data input into multiple systems.
- Working knowledge of Windows‑based applications such as Microsoft Office.
- 10 hours per day.
- Conduct comprehensive assessments of patients on admission and document in the EHR.
- Collaborate and communicate effectively with the primary nurse on patients’ admission needs.
- Ensure all home medication lists are accurate and up‑to‑date in the EHR.
- Orient patient and family to room, hospital, and nurse call system.
- Provide and document education to patient and families.
- Work in partnership with the throughput team to assist with early discharges.
- Collaborate and communicate effectively with the primary nurse on patient discharge needs.
- Provide discharge education to patient and family.
- Ensure all required admission and discharge tasks are complete prior to patient discharge.
- Document discharge instructions and education provided in EMR.
- Perform other duties as assigned.
- Frequent physical demands: sitting, standing, walking, pinching/fine motor activities, keyboard use/repetitive motion.
- Continuous physical demands: talk or hear.
- Lifting floor to waist 35 lbs.
- Lifting waist level and above 25 lbs.
- Accurate near vision, far vision, color discrimination, depth perception, and hearing.
- Anticipated occupational exposure risks: blood borne pathogens, airborne communicable disease, dust/particulate matter.
