Hopewell-Prince George Healthy Families is seeking a Family Services Specialist to conduct standardized assessments with prenatal and post-birth families and refer families to needed services. You will market the program in the community, schedule home visits, and advocate for clients with local resources.
Responsibilities include referrals, timely paperwork, and case management with weekly supervision to ensure service plans meet family needs and safety considerations.
