Registered Nurse (Children's and/or Learning Disability
Purpose of the Role As a key member of our multidisciplinary team, you will deliver compassionate, person-centred and evidence based palliative care to children, young people patients, their families and carers. You will help us achieve our vision of excellence in specialist palliative care, acting as a role model and supporting colleagues to maintain the highest standards of practice.
Key Responsibilities- Being an Accountable Professional
- Work in full accordance with The NMC Code and maintain all professional registration requirements.
- Apply relevant legal, ethical and governance frameworks to ensure safe, compliant practice.
- Use and interpret research to inform and enhance clinical decision making.
- Communicate confidently and sensitively with patients, families, colleagues and external partners.
- Build and maintain therapeutic, professional relationships.
- Maintain accurate, timely and comprehensive clinical records using appropriate digital systems.
- Engage in reflective practice, supervision and ongoing professional development.
- Promoting Health and Preventing Ill Health
- Use skilled communication and strengthbased approaches to support people in making informed choices about their care.
- Provide clear, accessible information about illness, treatment options, care planning and endoflife decisions.
- Uphold best practice in infection prevention and control.
- Assessing Needs and Planning Care
- Assess the holistic needs of patients and families at end of life, including preferences for care and treatment.
- Develop personalised, evidence based care plans with clear goals.
- Work collaboratively with patients and families to review, evaluate and adapt care plans.
- Assess mental capacity and apply relevant legislation when supporting decision making.
- Identify safeguarding concerns and act promptly to protect vulnerable individuals.
- Providing and Evaluating Care
- Deliver compassionate, person-centred care that prioritises dignity, comfort, privacy and safety.
- Provide high quality endoflife care, including support for the dying person, their family, and the bereaved.
- Safely administer and optimise medicines in line with national and local policy.
- Coordinate safe discharge or transfer between care settings.
- Monitor and evaluate the quality of care delivered by yourself and others.
- Leading and Managing Nursing Care
- Understand and utilise the skills and scope of all members of the clinical team.
- Lead and manage the care of a defined group of patients, prioritising, delegating and coordinating effectively.
- Support the education, supervision and assessment of students and colleagues.
- Provide constructive feedback, promote reflective practice and contribute to a positive learning environment.
- Use digital systems responsibly to access, share and record information.
- Improving Safety and Quality
- Apply health and safety legislation to maintain a safe working environment.
- Recognise risks to care quality, including staffing and skill mix, and elevate concerns appropriately.
- Participate in audits, quality improvement initiatives and service development.
- Report and reflect on incidents, near misses and adverse events to support learning and improvement.
- Demonstrate resilience and support others in managing uncertainty and emotional demands.
- Coordinating Care
- Work collaboratively across sectors and agencies to ensure seamless, person-centred care.
- Support individuals with complex needs to maintain independence and avoid unnecessary interventions.
- Advocate for people who are vulnerable or have disabilities, ensuring equitable access to care.
- Make reasonable adjustments to assessment, planning and delivery of care.
- This role requires an Enhanced DBS check.
- Duties may evolve in line with service needs and professional development.
- Flexibility in working hours is essential.
