What are YOU looking for in your next nursing position?

Community Health Network, LTD (CHN) is looking for a Licensed Practical Nurse (LPN) OR Registered Nurse (RN) who wants to

1) provide care to someone in their home,
3) work closely with an Administrative Team that appreciates them.

Currently looking for Fri-Sun, hours are daytime with flexible on start/end times. 10-hours shifts but not limited to. Possible for more hours per day. Family is in need of any/all help so would be willing to accept 8-12 hours/day.

Key Responsibilities:

Person-centered care
Electronic Health Record experience is a plus!
Medication administration, updating orders and day-to-day care coordination.
Effectively communicate with supervisors and guardians
Dietary support and monitoring
Relevant education and training based on the individuals care plan
Complete all other duties as assigned

What we NEED you to have (not right away! But prior to our hiring you):

Certificate in nursing from an accredited nursing school
Licensed as an LPN/RN as issued by the State of Ohio-Registered on the State Nurse Registry
BLS CPR
High school diploma, High school transcripts, GED or equivalent
Driver's License
Two professional reference letters
Social Security Card
