LPN Peds Private Duty Days

Role Overview: The Home and Community Based Services Licensed Practical Nurse provides direct nursing care in patients' homes, following physician prescriptions.

• Deliver high-quality clinical services within legal nursing scope, adhering to the care plan and infection control protocols.
• Accurately document patient visits and clinical notes in electronic medical records and visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, physicians, and care teams.

• Current, unrestricted LPN licensure and CPR certification.
• Valid driver’s license, reliable transportation, or access to public transit.
• Ability to work effectively in diverse home environments, demonstrating good communication, organizational skills, and independence.
• Preferably, prior home care experience and flexibility in work hours.

Compensation ranges from $22.90 to $34.35 per hour, depending on experience and location. Benefits include comprehensive packages, incentives, and retirement options.
