Position: RN Case Manager Experienced (IKC)

Overview

Location HYBRID. Though this position is primarily remote, it requires the RN Case Manager to meet with patients in-person within an assigned territory. This can range from 2, 3, or 4 days/week. It is necessary the RN Case Manager is local or within a reasonable distance within the market. Coverage includes Hot Springs, AR and surrounding areas. Territory assignments may change based on business needs.

This position functions autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for ESRD and CKD patients. This role coordinates care among patients, families, physicians, providers, care teams, the community, and the Regional Operations Manager.

Responsibilities
  • Coordinate and facilitate quality, cost-effective care for ESRD/CKD patients across the care continuum.
  • Collaborate with patients, families, physicians, providers, care teams, the community, and Regional Operations Manager to coordinate services and transitions of care.
  • Travel within assigned territory to meet patients in person as required by role.
Qualifications
  • A.D.N. degree from an accredited school of nursing required; B.S.N. preferred; three-year diploma may be substituted for a nursing degree
  • Current RN license in the state of practice
  • Continuing education credits as required by the state of practice
  • Minimum five (5) years’ clinical nursing experience
  • Minimum three (3) years’ renal nursing experience preferred
  • Case management experience strongly preferred with ability to assess patient needs, develop and manage care plans, coordinate across interdisciplinary teams, and support patients through complex transitions
  • Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred
  • Data-driven approach to clinical, financial, and patient outcomes; experience with CQI techniques
  • Experience in healthcare performance coaching
  • Current CPR certification
  • Ability to adapt to new/electronic processes and productivity expectations (Capella implementation)
  • Current driver’s license with a positive driving record
  • Basic computer skills; proficiency in MS Word and Outlook
  • Proficiency with DaVita clinical software (Capella) within 90 days of employment
  • Home office with reliable internet connectivity (minimum 1 Mbps upload/download)
