License Practicing Nurse (LPN) Rehabilitation

Job Type: Full-Time, Days

As a Licensed Practical Nurse (LPN) joining our team, you will provide compassionate care to patients alongside a dedicated team. You will assess patient needs, administer treatments, and ensure patient comfort and safety in a collaborative environment focused on delivering high‑quality patient care.

How you’ll contribute
  • Contribute to the assessment of patients by collecting data for analysis by an RN.
  • Perform plan of care interventions, including specimen collection and wound care; may administer medication.
  • Document patient care provided.
  • Assist RN with development of plan of care.
  • Assist patients with daily living activities including personal hygiene, elimination, nutrition, and ambulation.
  • Monitor, document, and communicate patient status and responses to interventions with RN, physician, patient, and family.
  • Advocate for the physical and emotional well‑being of the patient.
  • Participate in performance improvement and continuing education.
  • Provide quality patient care and work flexible shifts/units as needed.
  • Maintain regular and reliable attendance.
  • Perform other duties as assigned.
What we’re looking for
  • Current state LPN license.
  • Basic Life Support certification required within 30 days of hire.
  • One year of experience preferred.
  • ACLS certification required within 90 days of hire.
  • PALS certification required within 90 days of hire.
