Location:
Houma, LA (Local or Traveler) Assignment Length: 13 Weeks
Schedule:
Full-time, Monday-Friday, 8:00 AM - 5:00 PM, 40 hours per week (30 units)
Qualifications:
HCHB, Oasis, and HH experience required
Responsibilities:
- Provide skilled nursing care to patients in their homes, ensuring high-quality and compassionate healthcare.
- Conduct comprehensive assessments, including OASIS, to evaluate patient needs and track progress.
- Develop, implement, and update individualized care plans in collaboration with physicians and other healthcare professionals.
- Utilize Homecare Homebase (HCHB) software to document patient care accurately and efficiently.
- Educate patients and families on managing illnesses, treatments, and preventive healthcare practices.
- Maintain compliance with all healthcare regulations, documentation standards, and agency policies.
- Travel to patient homes, ensuring timely and reliable care delivery.
Qualifications:
- Required:
- Current RN license or compact license in the state of LA
- 1-2 years of RN experience in a clinical care setting
- 2+ years of Home Health experience
- Homecare Homebase (HCHB) experience
- OASIS experience
- Proof of current CPR certification
- Current driver's license, reliable transportation, and insurance
- Preferred:
- Bachelor of Science in Nursing (BSN)
- CHPN certification
Education:
- Graduate of an accredited Associate Degree in Nursing (ADN) or Nursing Diploma program - Required
- Bachelor of Science in Nursing (BSN) - Preferred
