Description

Responsible for providing non-emergent primary medical care atone or more community health centers. This position can be used throughout the Tilman J. Fertitta Family College of Medicine, andcan be affiliated with UH Health Family Care Center, a federally qualified health center look-alike. This mid-level clinician willbe responsible for all patient care activities, including chroniccare management, acute care, and care coordination. This mid-levelclinician will practice comprehensive primary care medicineincluding the provision of women s health and some office-based procedures.

1. Provides primary care for patients and adjust therapeuticregimens as needed.

2. Completes medical assessments including history, physical, andsocial assessments

3. Formulates diagnosis based on data collected and within scope of practice.

4. Discriminates between normal, normal variations, and abnormaldeviations by specific screening tests and refers patients withabnormal findings to appropriate members of the health careteam.

5. Discriminates between normal and abnormal findings obtained byhistory, physical screening, behavioral assessments and, in collaboration with the designated physician, implements an appropriate management plan.

6. Orders appropriate diagnostic studies and executes suchdiagnostic and therapeutic procedures as the physician mayassign.

7. Provides health maintenance and anticipatory guidance.

8. Executes physician s orders in accordance with legal, ethical,and institutional standards. Administers medications and treatments as prescribed; write or renew medication orders.

9. Performs related duties as required.

MQ:
Requires application of advanced principles, techniques andtheory in a professional discipline or a thorough general business management knowledge. Knowledge of this level is typically obtained through a directly job-related Master s degree or equivalent formal training in a recognized field of specialization that is directly related to the type of work being performed. Requires a minimum ofthree (3) years of directly job-related experience.
