Certification Preferred:

Residency-Trained Integrated Pediatrics Pharmacist to join the team supporting the Pediatric Emergency Department (ED) and Pediatric Intensive Care Unit (PICU). This is a mid-level clinical pharmacy role on a 7 on / 7 off evening schedule (12 PM – 11 PM).
The pharmacist will be responsible for safe and accurate medication dispensing, clinical interventions, medication profile reviews, IV admixture oversight, and collaborative multidisciplinary support to ensure optimal pediatric medication outcomes.
Key ResponsibilitiesClinical & Patient Care Support
- Provide drug-related information and education to patients, families, hospital staff, and providers
- Perform clinical interventions including:
- Medication profile review
- IV-to-oral conversion
- Therapeutic drug monitoring
- Medication histories
- Participation in multidisciplinary rounds
- Document all clinical interventions appropriately
Medication Dispensing & IV Support
- Fill and label prescriptions for inpatient and outpatient needs
- Maintain medication profiles and identify drug-related problems
- Ensure safe and timely preparation/scheduling of:
- IV solutions
- Additives
- Sterile compounded products (as applicable)
- Dispense controlled substances and maintain inventory documentation and compliance
Leadership / Training / Department Support
- Supervise pharmacy technical support staff
- Mentor/train pharmacists, pharmacy students, and residents
- Assume department management responsibilities when Pharmacy Manager/Lead is absent
- Medication Use Evaluation (MUE)
- Process improvement programs
- Policy/procedure drafting and review
- Newsletter collaboration as needed
Safety, Quality & Compliance
- Initiate reports for medication errors/adverse interactions
- Follow all Memorial Hermann standards, policies, and procedures
- Support productivity, quality outcomes, and supply/time management expectations
- Promote continuous education and competency development
- Maintain a patient-centered approach aligned with Memorial Hermann service standards
Education: Graduate of an accredited School of Pharmacy
Licensure
- Active Texas Registered Pharmacist license
- Sterile Product Certification (required if compounding sterile products)
- Through in-house or accredited program
Experience
- Minimum 2 years of hospital pharmacist experience
- Knowledge of unit dose medication systems and IV admixture processes
