Responsible for reading patient charts to determine medical history, including diagnoses and treatment given. Using a set of established medical codes transcribe patient history used by both health care providers and insurance companies. Maintains protected health information (PHI) documentation in accordance with Health Insurance Portability and Accountability Act (HIPAA).

MQ:
Requires a basic knowledge of grammar, spelling, punctuation and simple mathematical functions like percentages, ratios, etc. as might normally be acquired through attainment of a high school diploma or GED. Requires a minimum of one (1) year of directly job-related experience.
